Crashform is an independent tool for preparing a shared accident document. Participants record details, review both sides, sign and download a PDF.

What the tool prepares

The actual accident location selects the document layout. The prepared markets use a European accident statement, a Turkish KTT layout, a Québec joint-report layout or an exchange-of-details record. The Québec selection also uses the accident postcode. Check the location and the final document before using it.

The exchange-of-details PDF currently uses English labels. It is a shared record, not an official state or police filing. PDF preparation does not complete your insurer’s claim process.

How these guides are written

Each country guide links to local insurers, industry organisations or public authorities. Sources were consulted on 8 October 2026. The guides combine those references with instructions for using our actual tool. Source links are references, not endorsements or partnerships.

No local insurer approval or independent professional review is claimed. Requirements can differ between insurers and circumstances. Consult the linked sources and your insurer for the current procedure.

Independent from existing services

This product is independent of Assuralia and its previously published Crashform app. It is also independent of France’s e-constat auto, Spain’s Declaración iDEA, SBM and Québec’s GAA online joint report. We do not imply affiliation or automatic submission through those services.

What to verify before relying on a document

Check the facts, signatures, required national fields and submission method with your insurer. Crashform does not decide liability or guarantee acceptance by every insurer. Keep your own downloaded copy and complete any separate notification required by the authorities.
