Accident information exchange

For Australia, Crashform produces an A4 record of exchanged details. Check any separate reporting requirements for your state or territory and your insurer.

Preparing a document is separate from filing a claim. Crashform does not decide liability or guarantee acceptance by every insurer.

Registration and driver information

Record the vehicle registration, sometimes called rego, and the correct driver and contact details. Check the insurance information separately: the driver may not be the policyholder. NRMA’s claim guidance lists the types of information an insurer may ask for; your own insurer may need additional details.

State and territory procedures

Check the official procedure for the state or territory where the accident happened. Creating an A4 PDF here does not notify the police or submit an insurance claim. Keep the actual accident location accurate even when a vehicle or driver comes from another state.

Keep a useful record

Review both vehicles, the time and location, the description and sketch before signing. Describe what you observed. Save your PDF and keep any separate evidence with your records, then ask your insurer how to provide it. Photo upload and automatic claims submission are not available in this tool.

FAQ

Before you start.

Does this notify my insurer?

Is this an official state or territory form?

No. This is an independent exchange-of-details record. Check the applicable official reporting instructions.

Do we need to speak the same language?

No. Each person chooses from the available languages. The details belong to the same report.

Do I need to install an app?

No. Crashform works in your browser. You do not need an account to start a report.

Will my insurer accept the PDF?

Ask your insurer which documents and submission method they require. Crashform does not guarantee acceptance by every insurer.

Does Crashform decide who is liable?

No. You record details and circumstances. The document does not decide liability or compensation.
