Accident information exchange

For the United States, Crashform produces a Letter-size record of exchanged details. It is not a state accident-report filing. Check any separate state and insurer requirements.

Preparing a document is separate from filing a claim. Crashform does not decide liability or guarantee acceptance by every insurer.

Car accident information exchange checklist in the US

Information exchange and official reports

State Farm’s accident guide separates exchanging information, documenting the scene and obtaining a police report. Crashform helps with the shared information record. Check your state’s official instructions and your insurer’s process for any reporting or claim steps that apply.

Driver, vehicle and policy details

Check the driver’s name and contact details, license information where relevant, vehicle plate and insurance policy details. The driver and policyholder may be different people. Keep each vehicle consistently assigned to A or B, including the description and sketch.

A Letter-size PDF for your own records

Save your copy and ask your insurer how they want supporting information submitted. This PDF is not a DMV form or a submitted police report. Record observed facts rather than assigning fault, and keep separate evidence safely. The tool does not upload photos or send a claim automatically.

FAQ

Before you start.

Is this my state’s required accident report?

No. It is an independent exchange-of-information record. Follow the relevant state and insurer instructions separately.

Does it submit my insurance claim?

No. Download the record and use your insurer’s claims channel.

Do we need to speak the same language?

No. Each person chooses from the available languages. The details belong to the same report.

Do I need to install an app?

No. Crashform works in your browser. You do not need an account to start a report.

Will my insurer accept the PDF?

Ask your insurer which documents and submission method they require. Crashform does not guarantee acceptance by every insurer.

Does Crashform decide who is liable?

No. You record details and circumstances. The document does not decide liability or compensation.
