Canada and Québec use different layouts here

For an accident outside Québec, the tool produces a Letter-size exchange-of-details record. Québec has a separate joint-report layout selected by accident location and postal code. Choose the actual location; your preferred language and home province do not determine where the accident occurred.

Provincial reporting is a separate task

The Insurance Bureau of Canada provides collision resources and claim guidance. Follow the instructions of your insurer and the relevant provincial or territorial authorities. A downloaded PDF is not a police report, collision-centre submission or filed insurance claim.

Organise the information you exchange

Check driver names, contact details, vehicle plates, insurance information, location and what happened. Keep A and B consistently assigned to the same vehicles. Save your own copy before following up with the insurer, and keep any separate supporting evidence in your records.

No. It records exchanged details. Check the separate procedure with your insurer and the relevant authorities.

Where is the Québec joint report?

Use the Québec page for its guide. The accident location and postal code still determine the format in the tool.

Do we need to speak the same language?

No. Each person chooses from the available languages. The details belong to the same report.

Do I need to install an app?

No. Crashform works in your browser. You do not need an account to start a report.

Will my insurer accept the PDF?

Ask your insurer which documents and submission method they require. Crashform does not guarantee acceptance by every insurer.

Does Crashform decide who is liable?
